12-Month Rolling SummaryMar 2025 β Mar 2026
π Overview
Over the past 12 months Echo recorded events across Auckland (Penrose) and Wellington sites.
Near miss events are the most frequently reported category β reflecting active hazard
identification culture. The QEHS Event Form (introduced January 2026) added Fire Events,
Injury β No Treatment Required, Internal & External Non-Conformance, and Positive Safety
reporting. Forklift-related incidents appear as a recurring theme across both sites.

Likelihood Definitions
- 1. Not Possible to Happen Again

The incident could reoccur, but only under rare or exceptional circumstances where existing controls fail. - 3. Unlikely to Happen Again
The incident could reoccur, but existing controls generally prevent it and it would be expected to occur infrequently. - 4. Will Likely Happen Again
The incident is expected to reoccur at some point because the hazard is present and existing controls are not fully effective. - 5. Will Definitely Happen Again
The incident is expected to reoccur frequently or imminently unless immediate and significant controls are implemented.
